Fix encoding corruptions and refine phone-form-dialog styles

- Fix 4 mojibake instances (UTF-8 to GBK to UTF-8 double encoding)

  - Comment for el-select icon (line 313)

  - Comment for margin reset (line 1006)

- Refactor phone-form-dialog CSS for better modal layout
